Similar to the CLAIM1, the 5144 form is an Excel template which simulates the Crown Court claim form.  A powerful extension page allows unlimited augmentation of the detail in any of the sections 2, 3, 4, 5A, 5B, 6, 7 and 8.  A form can have as many extension pages as are required.  All the details, be they entered on the main form or on an extension page, are automatically summarised on the summary page. We will dispatch the software by return, including media CD, manual(s) and invoice. We ask that you settle the invoice by return.
